The Brain Health Toolkit is a comprehensive and free resource that empowers older people to understand when and how to talk to their primary care clinician about changes in memory and thinking.

The toolkit was created by a team of experts at the University of California, San Francisco and the University of Southern California. It is supported by the California Department of Public Health, Alzheimer’s Disease Program. © 2024, funded under contract #23-10608.
